Claim procedures can vary by type of insurance and by the insurance company involved. If you have a claim, contact your insurance agent or the insurance company directly, as appropriate, for claim forms, insurance company contact information and procedures.

Employee Disability Income Insurance claim to file?

Des Champs & Gregory’s Financial Services Department will send you the appropriate Disability Income Insurance Claim Forms for the insurance company involved, or provide the phone number to contact the insurance company directly.

You as the employer will need to complete the Employer Form. Your employee will need to complete the Employee Form, and the employee’s doctor will need to complete the Attending Physician Form. Originals of the completed forms and appropriate medical and other documents should be mailed or faxed as directed to the insurance company. You as the employer and the employee should keep copies of the completed forms and attached documents.

Employee Life Insurance Claim to file?
The beneficiary listed on the deceased employee’s life insurance beneficiary form (most current one) should contact the employee’s Human Resources or Employee Benefits person or department for the appropriate form to file a life insurance claim.

If you as the employer do not have the appropriate life insurance claim form, you may call Des Champs & Gregory’s Financial Services Department: (941) 748-1812

Des Champs & Gregory’s Financial Services Department as requested will send you or the beneficiary the appropriate Life Insurance Claim Form for the insurance company involved, or provide the phone number to contact the insurance company directly. The beneficiary should mail the original of the completed form and an original death certificate to the life insurance company. The beneficiary should keep a copy of the completed form and attached documents for his/her records.

Employee Medical Insurance Claim?
A medical insurance claim usually is submitted directly to the insurance company. Your insurance agent can help you obtain claim forms and the insurance company’s contact information.
