Des Champs & Gregory’s Financial Services Department will send you the
appropriate Disability Income Insurance Claim Forms for the insurance company
involved, or provide the phone number to contact the insurance company
directly. The originals of your completed forms and appropriate medical and
other documents should be mailed to the insurance company. Keep a copy of the
completed forms and attached documents for your records.
The beneficiary listed on the life insurance beneficiary form may call Des
Champs & Gregory’s Financial Services Department: 941-748-1812Des Champs
& Gregory’s Financial Services Department will send him/her the appropriate
Life Insurance Claim Form for the insurance company involved, or provide the
phone number to contact the insurance company directly. The original of the
completed form and an original death certificate should be mailed to the life
insurance company. The beneficiary should keep a copy of the completed form and
attached documents for his/her records.
A medical insurance claim usually is submitted directly to the insurance
company. Your insurance agent can help you obtain claim forms and the insurance
company’s contact information.
